UIL and CSISD Athletic Paperwork

IMPORTANT INFORMATION

CSISD has a  new platform for managing all pre-participation paperwork, so it is imperative you read the entirety of this message to ensure that your child is prepared for the 2026-27 school year.

Each year, in order for your student to be eligible to participate in Athletics, Band, Cheer or Drill Team, there are two things that MUST have completed prior to the first day of participation in both in-season and off-season activities.

  • A physical exam and medical history.  The physical exam must be completed by a physician.

  • Rank One online paperwork.

ALL parents/guardians will need to create an account in Rank One to complete the required online forms.

    ***Once your child's physical is completed and uploaded into Rank One, the athletic training staff will review it for approval.

UIL Physical Exams

PHYSICAL EXAMS

Students are required to use the Pre-Participation - Physical Examination Form that is linked here and attached to this email.  
Per UIL rules, NO OTHER form can be accepted.

CSISD PHYSICAL EXAM DATES

CSISD Athletics will be hosting two $25 dates that are fundraisers for each campus' sports medicine program.  ALL proceeds will go towards the sports medicine programs to purchase the medical equipment and supplies that are used to care for our student-athletes, band, cheer and drill team members.

AMCHS students - April 29th 3:30-6:30pm
CSHS students - May 6th from 3-7pm

Middle Schools and Intermediate students can choose whichever date works best for them but due to construction, and Little Olympics being held at CSHS on the same day we HIGHLY encourage attending the AMCHS date.

Information will be sent home through your campus coach/program director on how to sign up for an appointment.

OBTAINING A PHYSICAL EXAM FROM A PERSONAL PHYSICIAN

If you prefer to see your family physician, please have them complete this Pre-Participation - Physical Examination Form.

Once completed, please upload the physical exam document directly into your Rank One account.

Hard copies should NOT be turned into the front office, counselors, nurse or your coach/program director.
