Physicals

In order for your student to participate in Athletics, Band, Cheer and Drill Team, the UIL and CSISD require a YEARLY physical exam and medical history prior to any participation, including try-outs and off-season workouts.  These physical exams are to be completed by a physician. 

They are also required to fill out the online paperwork before the start of participation

Physicals:
Students are required to use the Pre-Participation - Physical Examination Form provided or a copy of the form from the UIL website dated "2026" in the top right corner.  Per UIL rules NO OTHER physical examination forms can be accepted.  

We will accept physicals for the 2026-2027 academic year physician dated on or after April 1, 2026.  Physicals physician dated prior to April 1, will expire on August 1.

If you cannot make the CSHS event, A&M Consolidated will host an event on April 29th.  

If you prefer to see your family physician, have them complete the UIL approved physical exam form.  Once completed, please upload the physical exam document directly into your Rank One account.  Please do NOT bring hard copies to turn in.  Once the online forms are completed, they will automatically update in the Rank One system.  You can print forms for your records, however, CSHS does not need a hard copy of the forms.

When CSHS has your physical and online forms complete in Rank One, your athlete will be ready to participate.
